A Practical Approach to Apneic Oxygenation during Endotracheal Intubation

چکیده

Patients undergoing general anesthesia with endotracheal intubation are exposed to a brief apnea period that is usually well tolerated. Conventional preoxygenation techniques may not provide adequate time for safe airway management in patients at high risk for desaturation or when an unanticipated difficult airway arises. Continuous oxygen administration during the apnea period, termed apneic oxygenation, can be used in conjunction with traditional pre-oxygenation techniques to prolong the time to patient desaturation when tracheal intubation is attempted. Clinical studies have evaluated the efficacy of apneic oxygenation using multiple techniques, nasopharyngeal catheters, nasal prongs, modified laryngoscopes, and buccal oxygen administration, all proved to be effective at prolonging the duration of apnea, by delaying the onset of hypoxemia.

Randomized Trial of Apneic Oxygenation during Endotracheal Intubation of the Critically Ill.

RATIONALE Hypoxemia is common during endotracheal intubation of critically ill patients and may predispose to cardiac arrest and death. Administration of supplemental oxygen during laryngoscopy (apneic oxygenation) may prevent hypoxemia. OBJECTIVES To determine if apneic oxygenation increases the lowest arterial oxygen saturation experienced by patients undergoing endotracheal intubation in t...

Nasal Cannula Apneic Oxygenation Prevents Desaturation During Endotracheal Intubation: An Integrative Literature Review

Patients requiring emergency airway management may be at greater risk of acute hypoxemic events because of underlying lung pathology, high metabolic demands, insufficient respiratory drive, obesity, or the inability to protect their airway against aspiration. Emergency tracheal intubation is often required before complete information needed to assess the risk of procedural hypoxia is acquired (...

Improvement of gas exchange by apneic oxygenation with nasal prong during fiberoptic intubation in fully relaxed patients.

To evaluate the effect of apneic oxygenation with a nasal prong during fiberoptic orotracheal intubation (FOI) in fully relaxed patients, 46 patients, who underwent tympanomastoidectomy under general anesthesia whose intubation lasted more than 3 but less than 4 minutes, were studied. Changes of arterial oxygen tension (PaO2), arterial carbon dioxide tension (PaCO2) and vital signs were measure...
